I had reported this issue previously, along with others.
I just want to add this incase it may be helpful in any way.
My app logos have started coming back slowly, it seems every 5-6 days another logo will come back. Ive tried to determine what is triggering them to reappear but i cant find anything.

MDMedia0808 Is it possible those apps were updated? Either way, I've already written a solution and am just testing the results over the next couple of days. Fix should be submitted by the end of the weekend.
